Must know
  • Medical info is one shared record across every club the athlete belongs to, so an edit applies everywhere.
  • You can collect 1 to 5 emergency contacts, and each requires a name, phone, and relationship.
  • Duplicate emergency contacts (same name and phone) are rejected.
An athlete’s medical record holds allergies, medications, dietary notes, insurance details, and emergency contacts. Because it belongs to the person rather than a single club, it stays consistent everywhere the athlete trains. Where it’s captured. Medical information is collected from families through the medical form — during registration and checkout, and from the family’s own portal. Families keep it up to date, and because the record is shared, their updates flow through to every club. How you use it. Open an athlete and look at the Medical Information card on the Overview tab to review allergies, medications, dietary needs, insurance, and the listed emergency contacts at a glance. Emergency contacts. Your club decides how many contacts to collect, up to five. Every contact needs a name, a phone number, and a relationship, and no two contacts can share the same name and phone number. Which medical questions are asked, and how many emergency contacts are required, are configured in your club’s form settings — see the forms and waivers documentation.
