Must know
  • If your club collects emergency contacts, it can ask for up to 5, each with a name, phone number, and relationship — or it can choose not to collect any.
  • Duplicate contacts (the same name and phone) are rejected.
  • The portal prompts you for emergency contacts only when a program your athlete is enrolled in requires them — and if your club asks for more than one, it tells you how many more to add.
Emergency contacts are part of your athlete’s medical record, so you add and edit them on the same page.
1

Open the medical form

There’s no Medical information entry in the main menu. Open the Medical chip on the athlete’s card in your Athletes list — that page has an athlete selector at the top and an Edit medical info (or Add medical info) button. (You can also open the Medical Information section on an athlete’s detail page.) Scroll to the Emergency Contacts section — it reads Emergency Contact when your club collects only one.
2

Fill in each contact

Enter the contact’s Name, Phone, and Relationship. To add more, choose Add another contact — up to the number of contacts your club allows (5 at most).
3

Save

Choose Save Medical Information. If two contacts share the same name and phone number, you’ll be asked to fix the duplicate before it saves.
When a program your athlete is enrolled in requires emergency contacts and none are on file, the Medical information page shows a Missing emergency contact notice with a shortcut to add one; if your club asks for several and you’ve added some but not enough, it reads More emergency contacts needed and says how many more to add. It’s worth clearing so coaches always have someone to reach. Related: How do I add my athlete’s medical information? · How do I sign a waiver for my athlete? · What is the Uplifter parent portal?
