  • Your club must have it on. On a class page, Report incident shows only when your club has Incident Reporting turned on. An athlete’s profile always shows the button, but a report only goes through when the feature is on.
  • You need a permission. Filing needs the Create Incidents permission, which the Coach role doesn’t include by default.
  • It’s timed when you submit. The report is stamped with the time you tap Submit report. There is no field for when it happened.
Before you start. Check with an admin that your club has Incident Reporting on and your role has Create Incidents (“File and edit incident reports”). The button shows even without the permission.

Open Report incident

On a class page, tap Report incident in the header (Incident on a phone). On an athlete’s profile, tap Report incident at the top, and that athlete is already picked. The Report an incident dialog shows who it’s Submitted to: your club, plus the program when you start from a class.

Pick the athletes involved

Optional. Tap a name to add or remove it. Each athlete you pick shows their Allergies, Conditions, Medications, Dietary and Emergency contact in the dialog while you write.

Fill in the details

Category starts on Injury / Medical and Severity on Minor, so change them if they don’t fit. Location starts with the class’s facility name (if it has one) and is blank from a profile. A Zone list also appears when the class is booked into specific spaces.

Describe what happened

What happened? is required. The footer reads “Describe what happened” until you fill it in. If you file after class, write when it happened here, since the report is stamped with the time you submit.

Submit report

Tap Submit report. You’ll see “Incident reported”. The report is titled for you from the category and the athletes’ first names, and filed as Open for your club’s staff.
If your report is refused. The dialog stays open with what you wrote. “Forbidden” means your role doesn’t have Create Incidents, so ask an admin to grant it. A message that the feature isn’t available on your current plan means your club doesn’t have Incident Reporting on.
