# How do I file an incident report?

> Create an incident report with category, severity, involved athletes, and program or facility context.

<Note>
  **Must know**

  * Available on plans that include **Incident Reporting**, and filing requires the **Create Incidents** permission — only Admin has it by default.
  * Filing sends one internal email to staff who can view incidents; no one outside your staff sees it.
  * Coaches can file too — from a class roster or an athlete's profile in the coach portal — but only if you've granted their role the **Create Incidents** permission, which isn't part of the Coach role by default.

Log injuries, illnesses, behavior issues, facility problems, and near misses in one place, with the athletes and context attached.

  <Step title="Start a report">
    Go to **Athletes → Incidents** and click **Report Incident**.
  </Step>

  <Step title="Describe what happened">
    Enter a **Title**, pick a **Category** — Injury / Medical, Illness, Behavior / Conduct, Facility / Equipment, Near Miss, or Other — and a **Severity** of Minor, Moderate, Severe, or Critical. Set **When did it occur?** (separate from when you're filing), write a **Description**, and optionally type a free-text **Location**.

    Optionally choose one or more **Involved athletes**, and link a **Program** and **Facility**. Submit to create the report and open its detail page.
