# How do I choose which medical information to collect?

> Turn standard medical fields on or off, set how many emergency contacts to ask for and how long medical info stays valid, and add your own medical questions.

  **Must know**

  * **Where it lives.** Medical Forms is under **Settings > Athletes**, and only club admins can change it.
  * **Valid for 180 days by default.** You can set 1 to 3650 days. After that, athletes are asked to re-submit.
  * **Programs switch it on.** A program asks families for medical info only when its **Medical Information Requirement** is on.

**Before you start.** Staff with settings access can view this page, but only club admins can save. Anyone else sees an error such as **Failed to update settings**.

  <Step title="Open Medical Forms">
    Go to **Settings > Athletes > Medical Forms** to open **Medical Information Settings**.

  <Step title="Turn standard fields on or off">
    Under **Standard Questions**, switch each field on or off: **Allergies**, **Medical Conditions**, **Medications**, **Emergency Contact**, **Dietary Restrictions** and **Insurance Information**. While **Emergency Contact** is on, choose **Number to collect**, from 1 to 5.

  <Step title="Set how long medical info stays valid">
    Under **Medical Info Validity Period**, enter **Valid for (days)**, from 1 to 3650, then press Enter or click away to save it.

  <Step title="Add your own medical questions">
    Under **Custom Questions**, choose **Add Custom Question**. Enter the **Question**, pick an **Answer Type** (**Short Answer**, **Yes/No**, **Multiple Choice** or **Checkboxes**), add **Options** for the two choice types, and turn on **Required question** if families must answer. Choose **Add Question**.

**When changes save.** Switches and **Number to collect** save the moment you change them, with a **Settings updated** message. Questions save when you choose **Add Question** or **Save Changes**, confirmed by **Question added**, **Question updated** or **Question deleted**.

**What's on from the start.** **Allergies**, **Medical Conditions**, **Medications** and **Emergency Contact** (one contact) are on. **Dietary Restrictions** and **Insurance Information** start off.

**How validity works.** An athlete whose medical info was submitted or updated within the window isn't asked again during registration or checkout. Once it's older than that, they're asked to re-submit it.

**Managing your questions.** Drag a question by its handle to reorder it. Choose **Edit** to change it, or the trash icon and then **Delete** to remove it. Families stop being asked a removed question.

**Not the same as Custom Questions.** The **Custom Questions** card on this page is for medical questions only. The separate **Settings > Athletes > Custom Questions** page collects general registration questions.

**How this connects to programs.** This page asks families nothing by itself. A program asks for medical info at checkout only when its **Medical Information Requirement** is on (on the **Requirements** step), and then asks the fields and questions set here.
