# How do I add custom questions to registration forms?

> Collect extra intake information from athletes during registration with custom questions.

  **Must know**

  * Available on plans that include **Custom Information** — if you don't see it, ask your Uplifter contact.
  * Answer types are **Number**, **Yes / No**, **Signature**, **Short Text**, **Long Text**, and **Image Upload**. A Number question needs a **Minimum** and **Maximum** (the minimum is allowed to be 0).
  * A Yes / No question can **require a signature when answered "Yes"**, and any question can be given a **Validity Period** (1–3650 days) after which the answer must be re-collected at the next registration.

Custom questions collect information you need from athletes during registration — anything from a weight or shoe size to a consent acknowledgment. This is separate from waivers and medical forms.

  <Step title="Open Custom Information Questions">
    Go to your **Custom Questions** page and choose **Create Question**.
  </Step>

  <Step title="Write the question">
    Enter the **Question** text and, if it helps, an optional **Description** with extra instructions.
  </Step>

  <Step title="Choose an answer type">
    Pick the **Answer Type**. For **Number** questions, set the **Minimum** and **Maximum** and optionally allow decimal values. For **Yes / No**, you can require a signature when the answer is "Yes".

  <Step title="Set required and validity">
    Toggle **Required question** on or off, then choose a **Validity Period** — *Indefinite* so it never expires, or *Expires after a set number of days* so athletes re-answer once it lapses.

  <Step title="Save">
    Choose **Add Question**. It's now collected during registration; edit or remove it any time from the same page.
